1. A set of copies of Health Certificate, Seafarer^,s Identity Document, Seafarer^,s Passport, passport and certificate of job qualification(deck officers of deck department also need the copy of GMDSS Certificate).

1. A set of copies of two-year Health Certificate, Seafarer^,s Passport and certificate of job qualification.
